During a sightseeing flight the weather abscured the pilot to fly the route. The helicopter landed in mountainous terrain on the southern slop of Mauna Loa Mountian and wait for the weather conditions to improve. After about 2 hours the pilot attempted to restart the helicopter, however, the battery did not have enough pawer to start the engine. The pilot called for another company helicopter to retrieve the passengers.

The second helicopter (N279F) picked up the passengers and during the takeoff in a downslope direction, the pilot reported the he encountered a dowmdraft. A landing gear skid struck terrain and the helicopter rolled over. A small potsrash fire was extinguished by the pilot. TTAF: 7868

The helicopter was about 125 feet AGL in a climbing left turn when the pilot heard a loud bang. The pilot entered autoratation. The pilot noted that the main rotor RPM was low and was unable to regain any main rotor rpm before the landing flair. At touchdown, the main rotor blades flexed downward and severed the tailboom. The 3rd stage compressor had fialed, damaging the remaining stages of the compressor. The compressor case plastic was eroded. The engine had undergone major maintenance at 1774.5 hours which required the removal of the turbine section. There was no evidence inthe eng. logbooks of the compressor section being inspected at 1750 hours as required by the manufacturer. reported engine stoppage and crashed in the Western Tiers area of northwestern Tasmania. The helicopter was being used in support of the Tasmanian National Parks, including external sling load
operations. Following delivery of an external load of coal, the helicopter was reportedly landed, external nets gathered, and three passengers picked up. On short final to the passenger delivery,
as the helicopter was about 150 to 250 feet above ground, the engine reportedly flamed out. The pilot entered an autorotation and the helicopter impacted hard. The pilot and three passengers

301199 dam N5238C During a forced landing after an engine failure, the helicopter struck a standpipe, shearing its cross tube, and rolling onto its side.
The pilot was in the process of picking up an 800-pound sling load on a 100-foot-long line. As his load reached about 90 to 100 feet
agl, the engine quit. He released his load, turned to the right to avoid people and equipment on the ground, and entered autorotation. The conditions at the accident site were dusty and gritty. The engine teardown revealed that a second stage compressor vane had failed in fatigue, which resulted in a catastrophic failure of the compressor section. All first stage vanes were intact, and all third, fourth, fifth, and sixth stage vanes exhibited some degree of foreign object damage (FOD). The plastic coating on the first and second stage vanes exhibited erosion that exceeded the maximum allowable limits specified by the Rolls-Royce Allison 250-C20 Series Operation and Maintenance Manual. The first stage compressor blades were eroded, with the leading edges rolled over and scalloped. Metallurgical examination of the failed vanes revealed that they met all chemical and manufacturing process specifications. According to the maintenance records, a 300-hour inspection had been performed on the engine 130.1 flight hours before the accident. The manufacturer recommends a compressor case inspection be accomplished every 300 hours when operating in a corrosive environment. No evidence was found

The helicopter had an external load flight and crashed in the vicinity of Florida Power Corporation, Intercession City sub station, Florida. The pilot was hovering out of ground effect at about 75 feet above ground level adjacent to power lines to the left of the helicopter. The helicopter started to sink. He applied right antitorque pedal and the engine noise decreased. He moved the cyclic to the right, increased collective pitch to arrest the rate of descent, and the engine out audio activated. As soon as he cleared the power lines, he lowered the collective pitch to gain rotor rpm, and applied forward cyclic in an attempt to clear a tree to his front. He realized impact with the trees was imminent, and applied aft cyclic in an attempt to make a vertical descent. The helicopter collided with trees and terrain hard in a nose low left skid low attitude. Examination of the main fuel tank revealed the fuel tank was empty and not ruptured. The engine was removed, transported to the engine manufacturer, and placed in an engine test stand. The test revealed that the TOT indicated on the cockpit gage was below actual engine outlet temperatures, and that lower than actual TOT readings provided to the pilot via the cockpit TOT gage may have resulted in numerous operations being conducted in higher temperature ranges than that authorized by the Pilot's Flight Manual and/or the engine manufacturers temperature limitations.
